creating a custom letter

Hi,

I was wondering if someone can explain how to make a custom report off of one entry. Like for example I want to generate a simple report that I can copy and paste out into Microsoft Word or an email and it can have certain fields from the entry put into it. Basic example below. Any help would be greatly appreciated! (still trying to get the hang of using hooks) If i didnt explain that well please feel free to ask follow up.
