Is there a resource that will help my customize /table_view.php form? I am trying to get it to look like my existing form that we are using

assuming that you have all those fields on that page, you can use customize the templates/table_templatesDV table_templatesDVP to your needs. Just remember that if you recreate your app those files will be overwritten, unless you make the read only.

Another solution would be to customize your page via javascript to meet your need.
