app generation dialog enhancements

It would be nice in the app generation dialog, in addition to having checkbox selectors on individial files, to have two additional options.

1. A button on the dialog for "Select all and build". For smaller projects (1-20 tables) it's fast enough to just build everything.

2. Options in the build dialog (checkboxes, although messy for large projects, so maybe a multiselect list) to select/deselect "files" by table name. For example, if all I've modified is the Employee table, it would be much simpler to deselect all tables then click on Employee - rather than sifting through the lists to find just the Employee files.
