5.97 insert sometimes (!) fails

Hi,
I have a simple table....sometimes (I did not notice before) it's not possible to add a new entry even though it should be no problem.
Please see the attached images for table definition and AG error. Unfortunately I can not say, what makes AG allow the addition and what breaks adding the new record. It seems random.
